Could someone please translate this for me in english: "pinche...... como sigues" Thank you

I think pinche is a term of endeerment, like buddy.

And como sigues -or como sigues? perhaps- is a less formal alternative for Como estas?

My best translation would be: Buddy, how're you doing?/how are you?/what's up?

"Pinche" could be translated many ways. The speaker is probably greeting a friend with an insult, just as we do in English with friends.

Numb nuts, how are you doing?

You did not provide context, but I can imagine hearing the exchange between two male friends who have not seen one another for a long time, a guy visiting a friend in the hospital, etc. It's almost guaranteed that the speaker is Mexican. In Mexico "pinche" or "pinchi" is normally used as an adjective to mean "damn, useless, crappy, lousy, etc." "Pinche" is heard in Nicaragua, but the meaning there is "stingy, tightfisted, etc." You'll notice that the speaker is using "tú". This may or may not mean a relationship between peers. It could also mean that the speaker is pissed off. Context is needed...
