CERTIFICO: Que en el registro de nacimientos de: "nombre de ciudad"
I CERTIFY : That in the registry of births of: "city name"
That looks like a correct literal translation to me. Is that what you are looking for?

No, I am looking for the translation that means the same in English as in Spanish.
I'm not sure I understand. Both the English and Spanish, as you have written them, mean the same.
Such a document in English would probably be constructed differently but your translation of the Spanish is correct.
Do you want to alter it so that it appears to conform to an English style document? If so, that would require some changes. Is it from a legal document or from a letter?
Are you a native English or native Spanish speaker?

I guess what I ultimately need is for the translation to be understandable to a native speaker of English.
My native language is Spanish, so I know sometimes the translation can look correct to me, to a native English speaker is not understandable.
