Depending on the context, it could refer to a given transaction outside the stock exchange.

THe English equivalent is "Over-the-counter" or OTC as it is usually abbreviated.
There are different types of stocks that are traded in different areas, and these are NOT on the large stock markets (NYSE, NASDAQ, etc.) because they don't meet the requirements (yet) to be "publicly" traded. You can still buy them, your broker just has to have access to them, as opposed to the NYSE or NASDAQ where anyone has access to them.

It's for those stocks that are NOT traded on the big exchanges, like the NSYE or NASDAQ.

"Mercado extra bursátil" is used as the opposite for "mercado bursátil"...
Correcto, por eso hablar de acciones cotizadas en un mercado extra bursátil es hablar de acciones cotizadas fuera de la bolsa de valores (outside the stock exchange)

I agree with jlrussell, it's the OTC market (over-the-counter).
I still have some doubts about this. An OTC market is a stock exchange where securities transactions are made via telephone and computer, but is still through a stock exchange. Yet the term “extra bursátil”, seems to imply that the transaction has been made through a parallel market (shares not listed in the stock exchange). I could be wrong, but that’s the impression I get from the term.
