In my opinion, it means If someone won your heart once, no matter what happens, he/she will forever "own" you.
It may sound a little less romantic than your translation reminder, what do you think?
If that's what it means then the author must not have lost his/her heart but once. Not so in my case. Some of us have lost our hearts a few times...

I think this saying is for romantic people. And we all know spanish is/sounds more romantic than english. Maybe that's why there's no correct translation.
