Me suena mejor "dolor constante" pero lo he buscado en el DRAE.

constante
3. adj. Dicho de una cosa: Persistente, durable.
4. adj. Continuamente reiterado.

Seguí tratando de buscar alguna diferencia sutil, muy sutil, entre el concepto de "permanencia" y "constancia" y mira mis resultados:
permanencia
1. f. Duración firme, constancia, perseverancia, estabilidad, inmutabilidad.
