Thread: HELP - MEDICAL Translataion

I am translating some medical documents and I have a sentence that is killing me:

Exploración cognitiva: Cuestionarlo Frontal Scale Behaviour y DEX (rellenados por su ex-mujer), Torre de Londres, Dígitos, TAVEC, cowat, tarea de denomincación viso-verbal, praxias melocinéticas, tareas de lectoescritura y cáculo.


This is what I have so far in english: Cognitive assessment: Question Frontal Scale Behaviour and DEX (filled in by his ex-wife), Tower of London, Digits, TAVEC, cowat, visual/verbal denomination task, praxias melocinéticas, and literacy and calculation task.

Any help anyone can give me will be much appricieated!!!
